Objective: The objective of this retrospective study was to report the clinical outcome of dogs with craniodorsal hip luxation (CDHL) treated with a modified prosthetic capsule technique (mPCT). Animals and procedure: Retrospective study of medical records from dogs with CDHL treated with mPCT between 2012 and 2018. Results: Sixty-four dogs were included. All luxations were reduced successfully. No intraoperative complications were observed. The postoperative complication rate was 28% (n = 18), with 14% (n = 9) major and 14% (n = 9) minor complications, and the majority occurring within 1 mo after surgery. Complications included reluxation (9.4%), severe lameness (3%), osteomyelitis (1.6%), and intermittent lameness or stiffness (14%). Of dogs without major complication (33/55 dogs), long-term owner follow-up assessment (> 6 mo) (median: 39 mo, range: 19 to 51 mo) revealed an excellent or good quality of life, although 5 dogs had intermittent lameness or stiffness. Radiographs of 13/33 dogs (median: 27 mo, range: 10 to 46 mo) showed no or mild osteoarthritis progression. Conclusion and clinical relevance: The mPCT is an effective technique for surgical treatment of CDHL in dogs. Further studies are required to objectively evaluate limb use and long-term radiographic changes.


Résultat et complications après une technique de capsule prosthétique modifiée pour le traitement de la luxation cranio-dorsale de la hanche chez le chien. Objectif: L'objectif de cette étude rétrospective était de rapporter les résultats cliniques des chiens atteints de luxation craniodorsale de la hanche (CDHL) traités avec une technique de capsule prosthétique modifiée (mPCT). Animaux et procédure: Étude rétrospective des dossiers médicaux de chiens atteints de CDHL traités par mPCT entre 2012 et 2018. Résultats: Soixante-quatre chiens ont été inclus. Toutes les luxations ont été réduites avec succès. Aucune complication peropératoire n'a été observée. Le taux de complications postopératoires était de 28 % (n = 18), avec 14 % (n = 9) de complications majeures et 14 % (n = 9) de complications mineures, la majorité survenant dans le mois suivant la chirurgie. Les complications comprenaient la reluxation (9,4 %), la boiterie sévère (3 %), l'ostéomyélite (1,6 %) et la boiterie ou la raideur intermittente (14 %). Parmi les chiens sans complication majeure (33/55 chiens), l'évaluation de suivi à long terme du propriétaire (> 6 mois) (médiane : 39 mois, intervalle : 19 à 51 mois) a révélé une excellente ou bonne qualité de vie, bien que 5 chiens avaient une boiterie ou une raideur intermittente. Les radiographies de 13/33 chiens (médiane : 27 mois, intervalle de 10 à 46 mois) n'ont montré aucune progression ou une légère progression de l'arthrose. Conclusion et pertinence clinique: Le mPCT est une technique efficace pour le traitement chirurgical du CDHL chez le chien. BACKGROUND: In December 2016, three cases of serogroup B invasive meningococcal disease, including two children from the same middle school (11 to 15 years old pupils), occurred in the department (administrative district) Côtes-d'Armor (Brittany, France). They were infected by a rare strain (B:P1.7-2,4:F5-9:cc162), covered by the 4CMenB vaccine (Bexsero®). Four months later, two cases due to the same strain occurred in a high school in the same area (15 to 19 years old students). In accordance with French recommendations, vaccination was proposed to students of both schools and to all individuals aged 11-19 years living or studying in the hyperendemic area. We describe these vaccination campaigns, from the alert to the impact evaluation. METHODS: The target population included 8884 people: 579 in the middle school, 2007 in the high school and 6298 in the community. In both schools, vaccination sessions were organized directly on site. In the community, teenagers were vaccinated by general practitioners. The vaccination campaign took place from May to October 2017. An active pharmacovigilance follow-up was set up to document adverse effects of the vaccine. RESULTS: Considering the whole target population, the vaccination coverage was estimated at 43% for 1 dose and 34% for 2 doses. Higher vaccination coverage was observed in the schools (79% in the middle school and 42% in the high school for 2 doses) than in the community (27% for 2 doses). The reported adverse effects were consistent with the safety profile of the vaccine and no severe adverse effect was reported. CONCLUSIONS: This vaccination campaign was the third one implemented with Bexsero® in France and constitutes a reproducible approach for future targeted vaccination campaigns. No additional cases of the same strain have occurred since the end of the campaigns in the area.

RESUMO

A 12-year-old spayed female domestic shorthair cat was evaluated for a 3-week history of abdominal distension. Chyloabdomen secondary to pancreatic carcinoma was diagnosed. The cat was palliatively managed using rutin and a low-fat diet. The etiology, diagnosis and management of chyloabdomen are discussed.

An eight-year-old male English Setter was referred for management of a dislocation of a cemented dual mobility canine total hip prosthesis that occurred four months after the initial surgery. Revision surgery showed that the dislocation was associated with fracture of the ultra-high molecular weight polyethylene liner. The dislocation was successfully reduced after replacing the liner. A dual mobility acetabular component is composed of a mobile polyethylene liner inside a metallic cemented cup. Chronic wear of the components of a canine dual mobility total hip replacement has not been described previously. The use of this type of implant is fairly recent and limited long term follow-up of the implanted cases may be the explanation. Acute rupture of a polyethylene liner has never been described in humans, the only case of rupture of a polyethylene liner occurred 10 years after implantation. The case presented here of rupture of the polyethylene liner of a dual mobility total hip replacement is a hitherto unreported failure mode in this model of acetabular cup in the dog.

An 11 yr old spayed female Labrador retriever was diagnosed with a right adrenal tumor. At surgery, adhesions to the right kidney were dissected, allowing the right kidney to be preserved. The tumor showed extensive invasion into the suprarenal vena cava. It was felt that thrombus removal via venotomy could not be performed. Instead, the vena cava was ligated caudal to the liver and cranial to the right renal vein. The neoplastic gland was then excised en bloc together with the portion of the invaded caudal vena cava. Hind limb edema had developed preoperatively and increased transiently in the first days postoperatively. The animal was discharged 6 days postoperatively with no other clinical disorders, and hind limb edema resolved over time. Histopathology identified a pheochromocytoma. The dog died 49 mo later. A neoplastic thrombus of the vena cava may require venotomy to allow thrombus removal. Occasionally, removal of the thrombus by venotomy may prove impossible. In such a situation, en bloc removal of the concerned portion of the vena cava may be performed with a good long-term outcome provided that gradual occlusion of the vena cava by the thrombus has allowed time for collateral circulation to develop.
